Are you not seeing the two options under the Custom Avatar heading on the Edit Avatar page?

there is no Custom Avatar.
only for Community Patrons?

For Custom avatars - that facility is available to Yoggie Patrons. Otherwise there's over 70 generally available ones on the site.
